Myocardial scintigraphy, the price of which is quite high, is prescribed only in extreme cases, according to the following indications: determination of angina pectoris with dubious ECG indicators; assessment of ventricular function
One other patient who had had a heart transplant for ATTR-V122I cardiomyopathy showed only soft-tissue uptake on 99m Tc-DPD scintigraphy. This extensive uptake in ATTR wt and ATTR-V122I amyloidosis, currently considered to cause mainly cardiac disease, may have a wider phenotypic spectrum than previously recognized.

To perform a perfusion scan of the heart, radionuclides are introduced into the human body – substances that emit gamma rays (one of the types of radiation). Click on photo to enlarge. There are several types of radionuclides. Myocardial scintigraphy is most often performed according to the following indications: Diagnosis of coronary heart disease and other heart diseases.

Cardiac scintigraphy is used to determine if coronary angiography is necessary or to understand if a patient can benefit from cardiac revascularization therapy to recover the ailing heart muscle. Hepato-splenic scintigraphy with 99mTc-S-colloid was performed in twelve patients with bacterial endocarditis. These images showed that the size of the liver varied from normal to a severe hepatomegaly, depending on the presence of congestive heart failure. Intrahepatic distribution of the radiocolloid was slighty irregular in all cases. Scintigraphy can be used to estimate ventricular ejection fraction as an indicator of left and right ventricular function. In first-pass method, technetium 99m pertechnate is injected as an IV bolus and its passage through the heart recorded every second. The change in radioactivity recorded with time is related to the ejection fraction.

What is heart scintigraphy?
It uses radioactive tracers that are injected into the blood to produce pictures of your heart. Doctors use SPECT to diagnose coronary artery disease and find out if a heart attack has occurred. SPECT can show how well blood is flowing to the heart and how well the heart is working.

The scintigraphic pattern observed in these patients with bacterial endocarditis can be easily differentiated from that caused by only congestive heart failure, which is similar to the observed in patients with cirrhosis of the liver and/or portal hypertension.

This document is intended to supplement other guidelines for cardiovascular stress testing, including the recently published guidelines for myocardial scintigraphy.45 The American College of Cardiology (ACC)/American Heart Association (AHA) Task Force on Practice Guidelines regularly reviews existing guidelines to determine when an update or full revision is needed. Guidelines for the Clinical Use of Cardiac Radionuclide Imaging were originally published in 1986 and updated in 1995. A stress myocardial perfusion scan assesses blood flow to the heart muscle when it is stressed.
